Primary Care Provider (PCP)

Mount Airy, NC | Full-Time

Role Summary: The PCP serves as the leader within the patient-centered medical home model, providing the full scope of integrated primary care services, including diagnosis, treatment, and preventative care to patients of all ages.

Key Responsibilities

  • Provide primary care services at our main clinic and from the SurryMedMobile.
  • Obtain patient history, perform physical evaluation, order/interpret diagnostic tests, and initiate a treatment plan.
  • Consult and refer with/to appropriate Specialty Physicians.
  • Work collaboratively with on-site licensed behavioral health case managers.

Requirements & Experience

  • Medical degree and completion of a residency program in Family Practice.
  • Must be licensed in North Carolina or eligible for licensure, and be Board-eligible or certified.
  • Preferred: A minimum of three years of experience as a Primary Care Provider is preferred.
  • Bonus: Fluency in Spanish will be considered a bonus.

CNA or CMA

Mount Airy, NC | Part-Time to Full-Time

Role Summary: Responsible for sharing daily nursing duties, and providing patient care under the supervision of a licensed nurse. This role is key in creating a positive atmosphere for our patients, volunteers, and staff.

Reports To: Medical Providers, Medical Director, and Nurse Care Manager.

Key Duties

  • Accurate documentation of vitals, medication, and the intake process.
  • Provide interpretation services for Spanish speaking patients during office visits.
  • Collect and prepare laboratory specimens and perform in-house point-of-care tests.
  • Assist providers during examinations and assist with the Medication Assistance program for eligible patients.
  • Handle patient calls, transmit prescription refills as directed, and follow up on test results after consulting with the provider.

Compensation & Benefits

  • Compensation will be discussed during the interview (Position Pay: TBA).
  • Position begins as part-time and transitions to full-time (minimum of 32 hours per week consistently).
  • Full-time employees are eligible for paid personal leave (PTO).
  • PTO accrual is 10.0 hours per month during the first year of employment.
